First Visit & Patient Forms

Your initial appointment will begin with a consultation explaining your diagnosis and treatment options. Usually, treatment can be done the same day as the consultation. However, a complex treatment plan, or specifically scheduling a consultation only appointment  will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your initial appointment:

  • Your referral slip and any X-rays that may have been provided to you by your general dentist.
  • A list of medications you are presently taking.
  • Your dental insurance information, if applicable. This will allow us to help you process any claims.

Please also print these forms, fill them out, and bring them with you to your first visit.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.  The age limit for treatment is 14 years old and over.

Please alert the office if you have any condition that requires a pre-medication for dental treatment.  This would include any artificial joints, heart valve replacement, or transplant.

X-Rays

Please bring any X-rays that were given to you by your referring dentist to your appointment.  Our office will also take X-rays for our records and for comparison, to help in the diagnosis process.
